What is Medical Weight Management?

Medical weight management is a clinically guided approach to weight that goes beyond conventional dieting.

At Star Aesthetic Centre, Dr Rajeev Bangalee conducts a comprehensive medical assessment to establish the factors contributing to your weight, and then determines which approach is appropriate for your individual health and goals.

Depending on your medical history, goals and clinical assessment, your plan may include prescription medication such as Mounjaro® (tirzepatide) or Ozempic® (semaglutide) — GLP-1 therapies — where these are clinically indicated and lawfully prescribed.

These medications may help regulate appetite, increase feelings of fullness and reduce cravings, and can support blood sugar control, when combined with sustained changes to diet and activity. They are not suitable for everyone, they carry side effects and contraindications, and they are prescribed only after assessment.

There is no single approach that suits every patient. Each plan is individual and monitored by Dr Bangalee to check that treatment remains appropriate and safe, and results vary from patient to patient.

What is a GLP-1 receptor agonist and how does it cause weight loss?

GLP-1 is a natural hormone that helps control hunger, appetite and blood sugar. GLP-1 medications mimic this hormone, helping you feel fuller for longer, reducing hunger and cravings, and supporting weight loss.

Common examples include semaglutide (Ozempic®) and tirzepatide (Mounjaro®).

Is Ozempic available in South Africa for weight loss?

Semaglutide (the active ingredient in Ozempic® and Wegovy®) is available in South Africa. Supply has been constrained due to global demand — Dr. Bangalee monitors availability and will advise on the currently available formulation at consultation. Alternative GLP-1 medications (including liraglutide and tirzepatide) may be available as alternatives.

How much weight can I expect to lose?

Published trials of GLP-1 receptor agonists report average weight loss in the region of 10–20% of body weight over 12 months among participants who remained on treatment. A trial average is not a prediction for any individual — results vary considerably with starting weight, metabolic profile, dietary adherence and activity levels, and some patients respond very little. Dr. Bangalee will discuss what is realistic for your own starting point.

Is the medication safe?

GLP-1 receptor agonists have been extensively studied and are approved by regulatory bodies including the FDA and, in South Africa, SAHPRA. They are not suitable for everyone, and they carry side effects and contraindications — nausea and other gastrointestinal effects are common, and more serious effects are possible. Dr. Bangalee takes a full medical history and assesses suitability before any prescription, and monitors you throughout. Whether any medication is appropriate and safe for you is an individual clinical judgement made at your assessment.

Will I regain the weight when I stop the medication?

Some weight regain can occur after stopping GLP-1 medications, as appetite and hunger may increase again. This is why we combine medication with nutritional guidance and healthy lifestyle changes to help you develop sustainable habits that support long-term weight management.

For some patients, longer-term treatment or a maintenance dose may be recommended, depending on their individual needs and medical assessment.

Do I need blood tests before starting the programme?

Yes. Baseline blood tests may be required before starting prescription weight-loss medication. These may include blood sugar (HbA1c), cholesterol, thyroid function, kidney function and a full blood count.

These tests help Dr. Bangalee assess your overall health, ensure the treatment is appropriate for you and provide a baseline for monitoring your progress.

Can a medical weight loss programme help with Type 2 diabetes?

Yes. Some medications used in medical weight management can also help improve blood sugar control in people with Type 2 diabetes, while supporting weight loss.

If you have Type 2 diabetes, your treatment will need to be carefully assessed and monitored by Dr. Bangalee, particularly if you are already taking medication for your diabetes.

Is this programme appropriate for post-menopausal weight gain?
